Meningococcal (to prevent Meningitis)
Ages
11 - 21
* For youth entering sixth grade who have not yet received the meningococcal
vaccine, and are at least 11 years of age.
* For college-bound students
who will be living in a dormitory and have not been vaccinated OR were
vaccinated at 12 and require a booster before entering college.

Tdap: Tetanus, Diptheria & Pertussis
Ages 11 - 64
*For adolescents and adults who have not received a dose of Tdap or whose
vaccination status is unknown - a single dose of Tdap is recommended.
* For adults who have not been previously vaccinated and have close contact with infants less than 12 months of age.
